Letter of Appointment

We hereby appoint Frank Risk Management (FRM) to act as our Insurance Broker until further notice. This Authority replaces and revokes any previous authorities given or implied, to any agent or broker previously handling our insurances.

We further authorise the disclosure of personal information:

1. (a)   to FRM regarding our previous and/or current insurance held by any party.
2. (b)   collected by FRM:
   1. (i)   to any insurer or reinsurer and any of their agents, and
   2. (ii)   to any bank, solicitor or finance provider for the purpose of obtaining or confirming finance arrangements.

We acknowledge FRM will not be responsible for any errors or omissions in our current insurance arrangements until such time as these policies fall due and/or a full risk analysis has been completed by FRM.

By signing this letter of appointment, we agree to accept FRM's Terms & Conditions.

Pursuant to the Privacy Act the following is brought to your attention:

Any personal information collected is to enable FRM to evaluate the insurance you seek. It will be held by or on behalf of FRM. You may request access to, and correction of any personal information held by FRM in accordance with the provisions of the Privacy Act 2020.

FRM works on a total transparency basis so will fully disclose to you, all income we receive for the placement of your insurance.
